Stroke Rehabilitation Program
Our Focus Is On You and Your Family
The focus is on you and your family at North Oaks Rehabilitation Hospital.
The team will work closely with you and your family during daily therapy
sessions. Family conferences will be held to answer your questions and
address any concerns you and your family may have during your stay, and
when you’re ready to leave the hospital.

Your Expert Team of Professionals
The admission process starts with a nurse who will assess the effects of
the stroke and other conditions you may have. Other experts may also provide
input regarding your application for admission. If you meet the requirements
for admission, you will work with a specially trained team to set goals
that help you reach your highest level of independent living.
The team includes:

- Physical Therapists
- Occupational Therapists
- Speech Therapists
- Social Workers/Case Managers
- Nutritional Specialists
- Respiratory Therapists
- Medical Management
- Rehabilitation Nursing.

Education and Support Groups
The Stroke Support Group of Hammond offers support, socialization and education
for stroke survivors and their caregivers. Meetings take place the last
Monday of every month from 12:30-1:30 p.m. in the Conference Room of North
Oaks Rehabilitation Hospital at 1900 S. Morrison Boulevard in Hammond.
It is co-sponsored by North Oaks Health System and Southeastern Louisiana
